We directly deliver Mental Health, Physical Health, Pharmacy and Clinical Substance Misuse in an integrated healthcare delivery model to provide effective and responsive care to improve wellbeing and support better outcomes in the future.

As a registered nurse you will provide high quality primary care services and a range of specialist interventions for offenders. At HMP Exeter we deliver a nurse-led integrated primary care service and operate a specialist practice-based clinical model of care.


Responsibilities

* Deliver high quality clinical care within the Integrated Healthcare Service for the prison, including acute & primary care, long term conditions management, enhanced assessment and diagnostic intervention services and health promotion activities.
* Carry out a range of duties including reception screening, emergency response, long term conditions management, planned care (e.g. running clinics) and enhanced assessment and diagnostic intervention services.
* Demonstrate robust expert clinical skills and provide evidence-based nursing practice to support patient care in a prison setting.
* Be part of the nurse-led integrated primary care service and contribute to a specialist practice-based model of care at HMP Exeter.
* Work within multidisciplinary teams comprising all healthcare staff and strategic partners, managing a single referral pathway.
* Undertake training to maximise uptake of screening, health promotion and prevention activity, including vaccinations.
* Allocate a named care co-ordinator for every patient with complex needs to ensure proactive, evidence-based interventions, health promotion/prevention activity and management of acute and long-term conditions using a stepped care approach.
* Develop and regularly review integrated care plans and comprehensive risk assessments tailored to needs within the establishment.
* Collaborate with the Clinical Lead, GPs and other clinicians to meet and review service needs.
* Participate in training, professional supervision, and annual appraisal as part of team practice.
* Contribute to evidence-based practice development and promote research where appropriate.
* Ensure implementation of patient-centred care planning tools and long-term condition registers across all prisons within the service.
* Work with a skilled multidisciplinary team and manage the referral pathway across the service.
* Undertake training to maximise uptake of screening, health promotion and prevention activity, including vaccinations.
* Ensure health promotion is embedded in every aspect of the service.
* Liaise with senior Trust personnel to share expertise and ensure collaboration with other directorates.
* Work positively and effectively in a challenging and potentially hostile environment, with awareness of health and safety policies and prison standards.
* Operate in accordance with Trust policies and guidelines and remain informed about relevant prison material (Standards, Standing Orders, Circular Instructions, Notices to Staff, Health and Safety).


Qualifications and Requirements

* Registered Nurse with eligibility to practice in England (NMC registration).
* Experience or interest in offender healthcare and an ability to operate within an integrated primary care model.
* Willingness to undertake National Security Vetting for prison work.
* Ability to work in a multidisciplinary setting and participate in supervision and appraisal processes.
* Flexibility to work in community and secure environment settings as part of NHS services.
