UCLH@HOME Deputy Sister/Charge Nurse
Job Summary
An exciting opportunity has arisen to join our UCLH@HOME team, a service provided by Whittington Health in partnership with UCLH. We are at the forefront of integrated care, providing innovative and transformational care that best serves local communities. UCLH@Home is an NHS service that supports patients to leave hospital once medically optimised and safe to be managed in the community, while they remain under the care of their consultant team.
Main Duties
As a member of our team, your role will include direct care delivery within the patient’s own home and the use of remote‑monitoring technologies. Working with colleagues from acute specialities, the emergency department and community services, you will provide a truly integrated care pathway for patients across acute and community settings, developing and expanding the service to deliver the best care possible for our patients in the best way for them.
* Provide care for patients who are transferred to the community from UCLH prior to the estimated discharge date.
* Take a leadership role, responsible for case‑finding, team co‑ordination and organisation of transfers home.

* RGN
* Current NMC Registration
* BSc or equivalent level of experience
