Main Responsibilities
As a Senior Diabetes Nurse Specialist, you will:
1. Manage a caseload involving high-level assessment, advanced diabetes knowledge, diagnosis, initiating treatments, and coordinating care with multidisciplinary teams, including mental and social care.
2. Work closely with primary care, community services, and acute trusts to ensure diabetes care aligns with national recommendations and supports wider changes in diabetes and nursing care.
3. Serve as an expert resource to the multidisciplinary team.
4. Drive change to develop integrated care pathways across primary care, mental health, social care, and acute trust providers.
5. Enable patients with diabetes to access appropriate services.
6. Deliver high standards of care, aiming to prevent unnecessary hospital admissions.
7. Work autonomously with patients, families, and carers, managing referrals based on needs and priorities.
8. Negotiate roles and responsibilities with patients and professionals, referring to other services as needed.
9. Establish local networks with health and social professionals and develop protocols for safe, effective diabetes community nursing services.

The Ideal Candidate
We seek a motivated, compassionate nurse who:
* Is a First Level Registered Nurse
* Has experience as a Diabetes Nurse Specialist in acute or community care
* Holds independent or supplementary prescribing qualifications
* Has a Diploma in Diabetes and ongoing professional development
* Possesses a degree, master's, or is working towards one
* Has highly specialized knowledge in managing diabetes and supporting health professionals
* Has significant experience in diabetes nursing and development within the specialty
* Has experience working within a multidisciplinary team
* Has recent experience in innovations and implementing change
* Can critically analyze data and understand complex diabetes management
