Qualifications
QCF level 5 in Health & Social Care
Personal Qualities
· Must have excellent knowledge of the Bridgend area
· Ability to work on own initiative with minimum of supervision
· Ability to lead and motivate others
· Good organizational skills
· Adaptability
· Good team working skills
· Competent numeracy and literacy skills
· Competent IT user
· "Can do" attitude
· Driving License and own transport
Main Duties
· Taking on new packages of care and building a successful team to provide care in the Bridgend area
· Carry out service users reviews
· Assessment of new clients
· Liaising with professionals e.g. Social workers
· Develop care and support plans within 7 days
· In the absence of care coordinator compile weekly rosters
· Provide support & motivation to team
· Staff supervisions and appraisals every 3 months
· On call on rota basis
· Delivering of outcome focused care to individuals
· Participate in training as and when required
· Update care plans every 3 months
· Managing the absence procedure
· Completing spot checks
· Attending meetings
· Supporting team with their QCF's
· Arranging staff meetings every 2 months
· Involvement of investigatory interviews as part of the disciplinary procedure
· Call monitoring 50%/50% on average, 20 hours in office 20 hours in community
