Clinical Practitioner - Rapid Response Team

Do you want to develop your clinical and leadership skills in a dynamic environment in a community setting?

Our Community Rapid Response Team based in Waltham Forest East London is expanding.

Urgent response community multidisciplinary team consists of nurses, allied health, social care professionals delivering health and social care 24 hours a day 7 days a week. The aim of the team is provided clinical and social care outside of hospital settings to people in their own homes within 0- 2 hrs in line with the national Urgent care standard.

This role will focus on providing clinical care to people in their own homes.


Main duties of the job

* To work collaboratively with health and social care professionals.
* To carry out health and social care assessments.
* To work as an autonomous practitioner and be professionally and legally accountable for all aspects of their professional and clinical work, within agreed national local and professional standards and guidelines.
* To accept referrals from other disciplines unsupervised and co-ordinate intensive and complex packages of care with other multi-disciplinary (MDT) agencies
* To use enhanced skills and knowledge to assess the physical and psychosocial needs and develop a comprehensive and concordant plan of care with the patient, their carers and the wider MDT as needed.
* To undertake risk assessments, on every visit, to ensure the safety of both staff and patients establishing the safety or otherwise of the patients ability to stay at home, or return home safely from another care setting, providing and arranging necessary nursing and supportive packages of care.
* To develop and implement evidenced based clinical/ therapeutic interventions based on best practice, specialist knowledge, experience and specific training to improve health outcomes and promote choice.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of basic salary, with a minimum of £4,551 and maximum of £5,735, pro rata for part time staff

Person Specification


Qualifications

* Registered General Nurse (Adults) / Registered Paramedic
* BSc Hons Community Specialist Practice or equivalent
* Nurse Prescriber/Non-Medical Prescriber or willing to work towards
* Evidence of continuous professional knowledge and skills
* Relevant post registration course and experience
* specialist Community Practice Teacher


Experience

* Demonstrate experience of Interagency and collaborative working across organisational boundaries
* Demonstrate experience of Teaching and assessing individuals and contributing to a positive learning environment
* Demonstrate experience of providing complex packages of care and innovative practice
* Proactive approach to managing conflict and challenging situations


Knowledge

* Awareness of NHS national and local Urgent Care Community Response policies, standards
* Awareness of the content and application in practice of the NMC /HCPC Professional Codes
* Demonstrate knowledge of the importance of disease management and rehabilitation in the community .
* Act as mentor for pre/post registration students on placement in the team.
* Demonstrate a person centred approach to delivery of clinical care in the community


Skills

* Proficient IT skills eg Microsoft applications and clinical systems e.g. RiO/System One
* Excellent interpersonal, communication and leadership skills
* Able to demonstrate a wide range of clinical skills and knowledge relevant to post. E.g clinical observations and interpretation of results
* Good organisational and time management skills and ability to prioritise clinical care
* Maintain accurate and contemporaneous records
* Effective management of team and prioritising workloads


Other

* To be aware and demonstrate the Trust Values
* To be able to drive and have access to a car
* Act as a positive role model within the team and delivery of care
