Position
We have an opportunity for a Nurse, Occupational Therapist or Social Worker to join our well established Early Intervention team as a Care Coordinator.
Overview
Early Intervention in Psychosis is an innovative and evidence‑based model. The team works with service‑users aged 14 to 65 who are experiencing first episode or early psychosis, and their families, for up to 3 years. Clients may also present with an at‑risk mental state.
Responsibilities
* Conduct a detailed initial assessment, incorporating the CAARMs assessment to identify at‑risk mental states (ARMS) or first episode psychosis (FEP).
* Develop a formulation to plan the client’s care over the next three years, using a recovery philosophy to collaboratively identify goals and needs.
* Support clients in addressing difficulties with housing, finances, vocational activities and social inclusion, working creatively with team members and independently.
* Identify and manage risk using the Steve Morgan tools, including positive risk taking techniques.
* Maintain accurate records in My Care Record (formerly CPA) and keep them at a good standard, utilising strong IT skills.
* Carry out duties as outlined in the job description.
* Use a vehicle for business purposes; be a car driver with access to a vehicle.
Qualifications and Personal Qualities
* Significant experience in community care coordination.
* Positive and enthusiastic attitude in a demanding role.
* Ability to work independently and as part of a team.
* Capability to structure own workload and diary, utilising a range of tailored interventions.
* Strong IT skills, particularly with My Care Record.
Required Authorisations
