What you will do:
– Conduct thorough assessments and implement treatment and discharge planning for complex and frail elderly patients
– Engage with the Discharge to Assess process, ensuring smooth and efficient transitions for patients
– Work primarily in inpatient settings, requiring hands-on involvement and a proactive approach
