Join to apply for the Band 7 Community Nursing Senior Sister role at HCRG Care Group based at Livingstone Community Hospital.
We are looking for a passionate and dedicated Band 7 Senior Sister to join our friendly and dynamic Community Nursing team. You’ll work closely with Specialist Nurses, Community Nursing teams and MDT Co-ordinators in the DGS locality as a highly skilled case manager delivering high quality care to patients. Essential to the role is supporting patients in improving their quality of life, promoting excellence of health and independence within the community, preventing inappropriate hospital admissions and facilitating early discharge, as well as providing education, support with self-management and health promotion. You’ll care for patients and their families in the way you believe is best, delivering our high service standards, sharing best practice, and actively improving the way we work.
Main Responsibilities
* Provide advanced clinical nursing support for patients with complex long-term conditions and/or elderly/frail, managing a caseload autonomously with a focus on high-risk patients.
* Collaborate with health and social care professionals to ensure continuity of care, aiming to reduce preventable hospital admissions and improve quality of care in patients’ usual place of residence.
* Engage with innovation, including tele technologies, to support management of patients; promote self-management strategies for patients and carers to reduce avoidable use of urgent services and improve outcomes.
The Ideal Candidate
* Registered Nurse with current NMC registration and a minimum of 2 years’ experience working with patients with long-term conditions and complex needs at Band 6 or Band 7.
* Strong communication and organizational skills with a flexible approach to delivering positive patient experiences.
* Involvement in quality improvements, research and development; commitment to clinical excellence.
* Confident using IT systems and Microsoft Office (Outlook, Word, Excel).
