A first contact practitioner (FCP) is an experienced physiotherapist with advanced skills to assess, diagnose, and recommend appropriate treatment or referral for musculoskeletal (MSK) problems upon a patient's first contact with healthcare services, such as when visiting their GP. The role involves progressing and requesting investigations to facilitate diagnosis and treatment decisions.
Main duties of the job
Core Duties
* Be the first point of clinical contact for patients presenting with MSK conditions or symptoms, providing initial assessment, diagnosis, and management options.
* Use advanced clinical practice skills and reasoning to support patients with complex MSK conditions, multiple pathologies, mental health, or pain management needs.
* Act as a link between primary, community, and acute services, ensuring an integrated care pathway that meets patient needs, including triage and management advice.
* Provide leadership and support for MSK clinical and service development within the PCN, including learning opportunities for the multidisciplinary team.
* Offer highly specialist advice on treatment options and contribute to MSK service and pathway development.
* Work independently to assess, diagnose, triage, and manage a caseload of patients registered with the PCN, prioritising effectively.

SynergyHealth PCN covers three practices in Gedling Borough, including Jubilee Park Medical Partnership in Carlton, West Oak in Westdale Lane, Mapperley, and Trentside Medical Group in Netherfield and Carlton. The PCN has a population of 36,000 patients and includes care coordinators, social prescribing link workers, an advanced nurse practitioner, and clinical pharmacists, working together to enhance patient care.
