What you will do:
* Conduct strength-based assessments to identify the needs of adult service users with learning disabilities.
* Develop person-centred support plans tailored to individual needs and preferences.
* Facilitate long-term support arrangements and regularly review these to ensure continued efficiency and suitability.
* Carry out Mental Capacity Assessments to ensure clients’ needs and rights are fully considered.
* Engage in Court of Protection work, advocating for your clients’ best interests.
* Collaborate as part of a multidisciplinary team providing efficient and personalised services to adults, carers, and families, ensuring key objectives and quality standards are met.
