Overview
The post holder will be a Pharmacy Technician, site band 5 criteria or willing to undertake ACT and Medicines Optimisation courses, working as part of the ward-based pharmacy team to optimise safe and effective use of medicines. Duties include working on wards and surgical units, taking and recording patient drug histories, assessing patients’ own medications, counselling patients on their medicines and supporting discharge. The role includes contributing to audit and review of safe medicines practice in clinical areas and supervising junior staff and trainees on a planned basis. The post holder will provide final accuracy checking support to the dispensary on a rotation basis.
Main duties
* Undertake or train to undertake a ward-based medicines optimisation service; document drug histories on admission (including allergies and adverse reactions) and assess patients’ own drugs.
* Organise compliance aids for individual patients where necessary; support discharge planning.
* Follow SOP procedures for medicines management and participate in departmental audits and pilot schemes.
Responsibilities

* Operate within ward-based and hospital pharmacy processes to optimise medicines use and safety.
Qualifications and Experience
* Registered Pharmacy Technician with GHPC.
* Pharmacy Technician Certificate through City & Guilds, Apothecaries Hall, BTEC or NVQ Level 3 in Pharmaceutical Science.
* Accredited Checking or Medicines Optimisation Accreditation; second accreditation underway (either Accredited Checking Technician status or Medicines Optimisation).
* Significant experience in a hospital pharmacy (essential); experience of Top Up systems (desirable).
