Job summary
We are seeking an experienced Consultant Psychiatrist to join our Bromley Mental Health HUB.
The substantive consultant will be expected to conduct new assessments and follow-up clinics, offer assessments for urgent cases, prescribe medication, perform home visits, and undertake Mental Health Act assessments as required. Working routinely with the MDT is an expectation. The role also involves reviewing triaged cases and recommending initial care plans for new referrals.
Main duties include ensuring care delivery aligns with the care/treatment plan, regularly reviewing individual needs, and modifying risks and care plans accordingly.
The Pathways of Care aim to provide a seamless service to help individuals and their carers resolve crises, promote recovery, and reduce relapse risk. Effective communication with primary care, acute services, community services, third and voluntary sector agencies, and other partners is vital once individuals are discharged from the team.
