About the Role
Malon Way Children’s Home operates 24 hours a day, 365 days a year, providing care for our young people. As a Hub Practitioner, you will play a vital role in delivering day-to-day care and interventions in line with individual Care Plans. You will:
1. Provide personal and practical support to young people, ensuring their needs are met.
2. Develop and implement effective action plans focused on reducing risks and building strengths.
3. Work closely with young people on the edge of care and care leavers (including post-18), supporting their transition into adulthood.
4. Act as a key worker, building positive relationships and resilience, and ensuring multi-agency support plans are adhered to.
5. Drive progress through solution-focused and restorative approaches, working collaboratively with professionals to improve outcomes.
What We’re Looking For
6. Flexibility, resilience, persistence, and commitment to best practice.
7. Significant experience working with vulnerable young people and understanding the impact of trauma, neglect, and early adversity.
8. Ability to build effective relationships and support young people’s development.
9. NVQ Level 3 in Caring for Children and Young People (essential).

10. You will work on a rota, including shifts and out-of-hours work (paid at enhanced rates).
