Responsibilities
* Work as part of a multi-disciplinary team across Tayside to provide profession-specific clinical specialist acute-level care for children and young people with a wide range of complex needs in their own home or home setting.
* Contribute to the development and delivery of a new care model to support early intervention, prevention of hospital admission and early discharge.
* Be part of a new service development and model delivery within Paediatric Hospital at Home.
Qualifications and Requirements
* Profession-specific degree and registration with the HCPC and the relevant professional body.
* Knowledge and expertise in working with babies, children and young people with complex needs.
* Experience in service development, improvement work and evaluation is desirable.
