An exciting opportunity has arisen for a full-time/ Part time Band 6Respiratory Nurse Specialist in the well-established Leeds CommunityRespiratory Team.The Leeds Community Respiratory Team is made up of Specialist Nurses,Physiotherapists, HOS-AR associate practitioners and clinical support workersproviding care and treatment for patients with COPD and other Respiratorydisease, supporting admission avoidance, chronic disease management, pulmonaryrehabilitation ,Home ward Respiratory and the home oxygen service.
Main duties of the job
Youwill provide care to patients predominantly with Chronic Obstructive PulmonaryDisease (COPD) through different pathways, such as home visits or Clinics forhospital discharge as well as chronic disease management.
The team run the HomeOxygen Service in Leeds which provides assessment and monitoring forrespiratory and non-respiratory patients requiring home oxygen.
We are looking for passionate, dynamic and flexible individuals with someexperience of managing respiratory patients in a community or hospital setting.
The individual is required to adapt to changing demands and workload tocontinue to deliver high quality care and demonstrate clinical decision makingworking independently or as part of the wider multi-disciplinary team.
The respiratory service is a 7day service operating from 8:30am to 4:30pm, dueto the nature of the role, it is essential that you are able to travelindependently across the city and have access to a suitable vehicle forbusiness purposes.

Job responsibilities
The post holder will beresponsible for their own caseload within their speciality area as anautonomous practitioner. In doing so, they will assess, plan, deliver andevaluate patient centred programmes of care to a specialist level ensuring thatthey practice within their sphere of competence and knowledge. The post holderwill work to professional and regulatory body codes, standards and guidance atall times ensuring that their practice is grounded in evidence basedtheoretical and practical knowledge.
