An exciting opportunity has arisen for a Social Worker to join our enthusiastic Home Safe Hospital Discharge Service working at Wansbeck General Hospital.
Whilst the post will predominantly be based at Wansbeck General Hospital, the successful candidate must be flexible to provide service cover at other hospitals across the County, particularly Blyth Community Hospital, Northumbria Specialist Emergency Care Hospital, and Newcastle Hospitals.
The post is for 37 hours per week on a permanent contract.
Main duties of the job
The Role: You will work alongside individuals, families, and carers, using a range of interpersonal and communication skills to assess complex psychosocial needs, set objectives, develop care plans, and deliver interventions to facilitate safe hospital discharges. You will complete Urgent Needs Assessments, Mental Capacity Assessments, and collaborate with external services to access necessary resources.
You will benefit from a good balance of autonomy and support, managing your caseload with regular supervision and support from your Team Manager.
It is essential that you understand the Care Act and the statutory duties of Local Authorities. We are committed to protecting vulnerable adults, and all staff are expected to share this commitment.
The salary range for this part-time position is £38,626 - £42,708, pro rata.
