The post holder will be a highly specialist resource providing clinical expertise in care, advice and support within the Community Mental Health Team. They will assist the Team Lead with Professional Nurse issues and Supervision. The post holder will lead and work within the multi-disciplinary team to ensure a seamless, coordinated approach to person-centered care, across the clinical pathway, and timely onward referral to other professionals and agencies as appropriate.
Responsibilities
* Develop non-medical prescribing practice in the community service
* Assess, plan and delivery of specialist care for identified needs in collaboration with clients and their families who have mental health issues
* Facilitate the continuing development of a co-ordinated approach to prescribing and dispensing across primary and secondary care and ensure that regionally agreed guidance and legislative frameworks are adhered to
* Assist in the development of clinical governance structures to ensure high quality and safe practice guidelines, policy and procedure development/implementation and training for all stakeholders
* Develop and provide nurse led clinics as appropriate to service needs to include the Implementation of the Physical Health Care pathway
Skills and Qualifications
* A professional qualification in Nursing (Mental Health) on Part 1 of live NMC register
* 3 years post registration experience in a Mental Health acute hospital/community setting, 1 year of which must be in a Community setting at Band 6
* Must be a supervisor and assessor under the Future Nurse/Future Midwife standards
* Hold current registration as an Independent / supplementary non-medical prescriber with the NMC or Be willing to undertake regulated course of study within 2 years of commencement
