Live Connected is an exciting branch of the Connected Health group. We provide quality care to adults with mental ill health, learning disabilities, dementia, and complex needs through supported living.
KEY DUTIES AND RESPONSIBILITIES
* Undertake daily living tasks as agreed with the service user, their family, and the professionals involved with the individual service plan.
* Work within the policies and procedures of Connected Living.
* Follow care and risk assessments specific to each service user’s needs.
* Complete Health and Safety checks within the service in line with policies and procedures.
* Be confident to lone work in line with lone worker risk assessments.
* Ensure safe medication delivery and ordering to the service users' homes.
* Provide service users with opportunities to express their preferences on how tasks are carried out.
* Maintain and promote choice and independence for service users.
* Manage service users' behaviors in the least restrictive way possible.
* Complete daily notes, handovers, and relevant documentation at the end of shifts, providing high-quality handovers to incoming staff.
* Work as part of a team and wider MDT to support clients' emotional and physical wellbeing.
* Develop and maintain professional relationships with service users and the multidisciplinary team to ensure positive outcomes.
