Advertisement Details The Social Care Worker – Rehabilitation Support is a key part of the Intermediate Care Team. The Intermediate Care Team is a service delivered jointly by NHS Shetland and Shetland Islands Council (SIC). The team is made up of: a Nurse Team Manager, Occupational Therapists, a Social Care Worker Practitioner – Rehabilitation Support and Social Care Workers – Rehabilitation Support. Referrals into the team are made via a professional involved in the individuals care, for example, social work staff, a GP, Community nurse, or Physiotherapist.
