Job Overview
This is an exciting opportunity for a 12 month internal secondment for an individual with knowledge and expertise in the assessment and management of persons living with frailty presenting acutely to the hospital, to enhance their competence and knowledge in the physical assessment and ultimate management of older adults presenting with frailty syndromes. The 12 month cover is most suited to an internal candidate with knowledge of the team and service, ready to gain experience at a senior clinical and leadership level of a band 7.
Main duties of the job
* Accepting referrals from front‑door colleagues and proactively seeking suitable patients in the Emergency Department to pull onto the SDEC unit.
* Undertaking Comprehensive Geriatric Assessments (CGA) for individuals living with frailty presenting with a frailty syndrome and using risk‑management experience to support same‑day discharge.
* Collaborating with the nurse consultant, consultant geriatrician, ACPs, frailty practitioners (physiotherapist, pharmacist), RNs and resident doctors to facilitate assessments in the Frailty SDEC.
* Working closely with community partners to deliver a joined‑up approach across the local Frimley Health and Care Integrated Care System (ICS).
* Providing advanced clinical advice, leadership and support, demonstrating high levels of clinical judgement, critical analysis and advanced decision‑making skills.
* Managing change in a fast‑paced environment, monitoring and improving standards across the Frailty SDEC through supervision, evidence‑based practice, clinical audit and education.
Detailed Job Description And Main Responsibilities
* Work closely with the consultant nurse, Frailty Advanced Clinical Practitioners, Frailty Practitioners, in‑reach GPs, resident doctors and the consultant geriatrician team to deliver effective, reactive services for patients with decompensated frailty, ensuring rapid treatment delivery, care planning and hospital admission prevention where appropriate.
* Provide high‑quality innovative clinical practice. Be proactive in clinical decision‑making, underpinned by advanced theoretical and practical knowledge, and demonstrate improved patient care outcomes.
* Co‑operate with consultant nurses for older people, frailty practitioners and in‑reach GPs to identify and manage patients with frailty, supporting them and their carers in developing supportive management plans.
* Undertake Comprehensive Geriatric Assessments for individuals presenting with frailty and their carers where needs are highly complex, planning, implementing and evaluating care delivery according to changing healthcare needs.
* Work autonomously using advanced levels of history-taking and assessment within multidisciplinary team guidelines and protocols.
* Take responsibility for the assessment, planning, implementation and evaluation of evidence‑based care, recognising individual patient needs and preferences without direct supervision.
* Utilise advanced clinical skills in assessment and/or formulate a diagnosis and/or treatment and management plan. Present the patient’s case to the consultant on post‑take ward round when admission is considered necessary.

Qualifications
Essential criteria:
* Current unrestricted registered Allied Health Professional / Registered Nurse
* Post‑registration experience through personal development and training courses focused on older adults / frailty / advanced practice skills
* Physical assessment qualification at level 7 study
* Evidence of delivering education and training in practice
* Willingness to undertake further training in specialist area
Desirable criteria:
* Teaching and assessing qualification / mentorship qualification
Experience
Essential criteria:
* Significant post‑registration experience at band 6 or band 7
* Experience working with older adults with frailty and/or experience working in an ED / Acute / SDEC setting
* Experience as a senior member of a team, demonstrating leadership qualities and supporting other members of the team
* Demonstrates ability to work independently within sphere of competence and effectively within a multidisciplinary team
Desirable criteria:
* Evidence of the supervision and development of members of a team
Skills & Knowledge
Essential criteria:
* Ability to demonstrate specialist clinical examination and reasoning skills to assimilate information to make a clinical judgement regarding diagnosis and intervention within individual scope of practice
* Knowledge and experience in risk management and admission avoidance in this patient group
* Demonstrates an understanding and insight into current practice issues relevant to the service
* Ability to communicate effectively at different levels of the organisation, including with staff, patients / service users, visitors or external organisations both verbally and in writing, in the exchange of highly complex or sensitive information
Desirable criteria:
* Experience in evaluating care using audit and undertaking quality and service improvement measures
