Work Pattern
The shift pattern for this position is Monday to Friday, 9 am to 5 pm. Hours of work will be flexible to cover between 8 am and 8 pm, 7 days a week. The initial pattern is Monday to Friday, but hours and shift patterns may extend as the service develops. Part-time and flexible working options will be considered.
About the Role
The Home First Response Service is focused on working collaboratively with acute and community teams to identify frailty and support patients to remain at home or in a homely setting. Hospital at Home provides urgent, short-term hospital-level care at home. The post involves developing rehabilitation care plans for patients under these services. We seek an innovative, experienced physiotherapist to join our team in Renfrewshire, supported by professional supervision within the HSCP structure.
The team aims to support frail patients with early discharge from acute settings and prevent admissions. The role involves working autonomously and as part of a multidisciplinary team, with opportunities for involvement in Quality Improvement and Service Development.
Team and Responsibilities
The Home First Response team is multidisciplinary, working across acute and community settings to manage frailty proactively and reactively. The postholder will be involved in early intervention, assessment, care planning, and ensuring seamless discharge and continuity of care by collaborating with community teams.
