Main duties of the job
The successful candidate will work with registered and non‑registered colleagues to provide flexible and timely social care support to patients referred to the service. The post holder will provide formal line management and supervision to designated Integrated Support Workers (ISWs) within the team, and support shift coordinators with day‑to‑day operational management of the non‑registered aspect of the Urgent Community Response.
Service hours
Service hours are 8am to 10pm, Monday to Sunday, with variable shift patterns offered.
Responsibilities & Qualifications
* Working with registered and non-registered professionals to assess patients requiring social care support in their own homes.
* Providing flexible and timely social care support to patients referred to the service.
* Providing formal line management and supervision to designated Integrated Support Workers (ISWs) within the team.
* Supporting shift coordinators with day‑to‑day operational management of the non‑registered aspect of the Urgent Community Response.
* Utilising existing clinical skills as required to support the team.
* Strong communication skills and the ability to prioritise a busy workload.
