Main duties of the job

Reablement - Empowering Independence

As a result of continued expansion in the reablement service, we are seeking an Assistant Team Manager to work alongside our existing leadership structure. This role involves leadership, collaboration, and making a real difference!

We are looking for a confident and passionate leader to help shape our Reablement service in Wiltshire.

Reablement is a short-term service that helps people stay independent. Led by therapists, it offers practical support, advice, and equipment to help individuals reach their goals using their own strengths and community resources.

As the Assistant Team Manager, you will guide a skilled team of therapists and social care practitioners, designing creative support plans, supporting people following hospital discharge to reorient to their home environment and routines, which may include new adjustments, connecting people with their communities, and ensuring services are delivered safely, effectively, and in line with professional standards.

Supporting the Team Manager, you will oversee service delivery, respond to queries including case scenarios, supervise staff, and promote integrated working with health partners, voluntary groups, and local services. You will also lead on quality assurance and staff development, including induction processes and performance management.

Job responsibilities

Specific duties and responsibilities:

* Deputising for the Team Manager during times of absence, ensuring oversight, coordination, and leadership of the team, providing direct line management and supervision, reporting as required to senior leadership. Use your own initiative while collaborating with the Team Manager, Service Managers, and Head of Service towards successful delivery of reablement across Wiltshire.
* Participate in line management and leadership activities contributing to the success of the team, including chairing meetings, planning team development activities, and arranging training.
* Provide formal and informal supervision and support to therapy and social care staff, addressing gaps in practice and training identified through supervision and appraisal.
* Have excellent knowledge and ability in managing complex cases, with a flexible, creative, and solution-focused approach, including rehabilitation programs and Moving and Handling solutions.
* Provide expert advice, support, and facilitate creative problem solving for team members dealing with customers with complex needs, including undertaking community visits.
* Engage in Continued Professional Development (CPD) activities to embed learning and promote evidence-based practice within the service.
* Ensure quality assurance by monitoring legislative and regulatory compliance (e.g., Care Act 2014, Mental Capacity Act), and contribute to standards and performance monitoring.
* Ensure assessments are conducted to high standards, enabling individuals to achieve their outcomes, and that long-term care is provided in line with the Care Act 2014 eligibility criteria.
* Assist the Team Manager in managing resources, including capacity, performance, and skill mix, to ensure work is allocated safely and risks are managed appropriately, supporting optimal outcomes for customers, including hospital discharges, referrals, and equipment provision.
* Assist in responding to complex and challenging complaints.
* Analyze and interpret performance data to support decision-making, and prepare for monthly performance meetings. Focus on departmental priorities for high-quality services, continuous improvement, and meeting key performance indicators.
* Contribute to health and safety monitoring within the team, including lone working.
* Support policy and procedure development and monitoring within the service.
* Work collaboratively with partners, including council services, health & wellbeing boards, NHS services, voluntary sectors, and independent providers.
* Facilitate community support in compliance with CQC regulations, working proactively with providers.
* Support HR processes such as recruitment, induction, performance management, and managing sickness absence.
* Respond to civil emergencies, including managing a rest centre.
