We are recruiting a highly motivated, innovative and enthusiastic Primary Care Staff Nurse/Pharmacy Tech to join our Health & Justice team at HMP Bronzefield. By joining the Prisons Health Care Team, you will help meet the healthcare needs of patients in secure environments and promote health and well-being. The service operates on a 7 day week model to ensure high-quality care at all times.
Our motto is "Caring NOT Judging". We work in partnership with the criminal justice system and other agencies to provide high-quality primary care and mental health services to those who have offended or are at risk of offending, to improve health and social inclusion.
Main duties and responsibilities
* Provide an accessible individual patient assessment, treatment and referral service
* Monitor the ongoing health status of patients and address identified issues
* Influence patients’ health outcomes by addressing current lifestyle patterns/choices
* Implement effective discharge plans for transfer or release
* Co-ordinate multi-disciplinary clinical practice to provide integrated care and treatment programmes
* Work collaboratively with staff from other disciplines to provide integrated and coordinated treatment plans

Responsibilities (full job description and personal specification)
* Triage patient needs in accordance with SOPs and national policy (e.g., NICE)
* Assess, plan and implement care for patients within each pathway to promote wellbeing
* Facilitate evidence-based practice to ensure effective clinical care
* Carry out general nursing duties
* Maintain contemporaneous records
* Support and contribute to the development of integrated healthcare
* Act as a preceptor/mentor as appropriate
Qualifications and requirements
* Essential: Registered Nurse (Adult)
* Essential: Numeracy skills for safe administration of medicines
* Desirable: Evidence of relevant Continuous Professional Development
