This is an exciting opportunity for a Practice Nurse to join our team.
We provide excellent primary care and innovative services to a diverse city centre patient population of around 9,200 patients. Our practice is a supportive place to work and we value our dedicated team which currently consists of 3 GP Partners, 3 Salaried GPs, a Physicians Associate, Clinical Pharmacist, Physio, Practice Nurses, Health Care Assistant plus our management and admin team. We are active members of our Primary Care network, Heeley Plus. We work with people experiencing homelessness and there are opportunities to develop skills working to tackle health inequalities. We are an active training practice with GP registrars and Medical Students.
This is a full‑time position, 37.5 hours, but there is flexibility for condensed hours, job share etc. We are open to two part‑time positions.

Main Duties of the Job
We are seeking a motivated, compassionate, and skilled Practice Nurse to work as a member of the Practice Nursing Team and be responsible for acting within professional boundaries to provide care for the presenting patient including screening, clinical assessment, diagnosis, treatment and follow up of care; demonstrating safe clinical decision making and expert patient care. To be involved in all areas of Chronic Disease Management (Asthma, Diabetes, COPD etc.).
The Practice Nurse will work collaboratively with the clinical team which will include GPs and other health care professionals.
Job Responsibilities
* Assist in and perform routine tasks relating to patient care
* Undertake and promote cervical cytology
* Provide a comprehensive family planning service
* Manage newly diagnosed patients with Asthma, COPD & Diabetes
* Undertake Chronic disease management and prevention (Asthma, COPD, Diabetes, Hypertension, Hypothyroidism, IHD, Stroke & LD)
* Perform wound care using aseptic technique and general wound care
* Undertake opportunistic screening: BP checks, height, weight, BMI, urinalysis, glucose monitoring and smoking status
* To undertake phlebotomy
* Administer injections under an individual prescription or patient group direction
* Offer a holistic approach to travel health including comprehensive advice for patients prior to travel (NHS)
* Undertake ear syringing
* Support patients and colleagues in adopting sound infection control measures
* Competent in anaphylaxis and resuscitation techniques
* Child Health and Child Protection
* Promote health and well‑being using health promotion, health education, screening and therapeutic communication skills
* Refer and use social prescribing teams
* INR monitoring
* To supervise and assist junior members of the nursing team, NAs, trainee NAs, HCAs
* Undertake infection control measures, including audits and annual reports
* Adhere to the Nursing and Midwifery Council (NMC) Code of Professional Conduct and Scope of Professional Practice
* Ensure professional practice adheres to organisational policies, procedures and guidelines
* Maintain a professional manner
* Promote and maintain own personal development and clinical excellence
* Participate in organisational decision making, interpret variations in outcomes and use data from information systems to improve practice
* Promote evidence‑based practice using the latest research‑based guidelines and the development of practice‑based research
* Monitor the effectiveness of clinical practice through quality assurance strategies such as audit and peer review
* Participate in continuing professional development to ensure up‑to‑date evidence‑based knowledge and competence in all aspects of the role is maintained
* Regularly participate in clinical supervision as agreed
* Communicate effectively with patients and families regarding information about their care
* Provide advice and information to patient, carers and families where appropriate
* Communicate effectively and work collaboratively with medical, nursing and allied health professionals to ensure delivery of a coordinated service
* Ensure accurate documentation and record keeping in line with local policies and NMC guidelines
* Promote and demonstrate effective communication networks within the organisation and with partner organisations and other service providers
* Complete records, audits and reports and respond to appropriate questions and requests
* Help develop protocols and assist in audits and research
* Attend meetings as requested
* Support effective communication channels within the whole team
* Work according to Clinical Governance and support the Clinical Governance agenda

* Undertake any SEAs that you have been involved in
* Work with the management team to investigate adverse clinical incidents and complaints

Person Specification
Qualifications
Essential
* Registered General Nurse, currently registered with the Nursing & Midwifery Council, with a valid pin and no restrictions
Desirable
* Willingness to undertake further development opportunities
* Experience of working with vulnerable patients
* Experience of working in areas of deprivation
