Community Engagement Practitioner

to play a vital role in our

Enfield VCS

team. Your Role: What Will You Be Doing?

As a Community Engagement Practitioner, you will be an integral part of a multidisciplinary NHS team, collaborating with clinicians, social workers, and community partners to support adults facing moderate to severe mental health challenges. Your responsibilities will include: Managing a caseload of service users, serving as their key point of contact. Contributing to care planning, progress monitoring, and discharge support using the RiO clinical records system. Co-developing person-centered recovery plans that prioritize social goals and community integration. Building strong therapeutic relationships through trauma-informed and strength-based approaches. Helping service users access local resources, attend appointments, and engage in well-being activities. Promoting recovery-focused communication and advocating for co-production and integrated care. Liaising with statutory and voluntary sector organizations for effective service navigation. Attending clinical meetings and community events, representing Hestia and the Community Mental Health Team. Maintaining accurate documentation of all support activities and risk assessments. Adhering to safeguarding protocols and quality standards. What Do You Need to Bring?

of the areas we support, so feel free to share this in your application. Key Qualifications:

NVQ Level 4 in Care (or equivalent) or at least two years of experience in a mental health setting. Strong understanding of mental health issues, recovery principles, and co-production. Experience collaborating with professionals, services, and community partners. Knowledge of care planning, risk assessment, and relevant legislation (e.g., Mental Health Act). Excellent communication and relationship-building skills. Proficiency in IT systems, including electronic case management tools.
