Overview

Central London Community Health NHS Trust is responsible for delivering Brent Adult and Children's Community services. We are seeking enthusiastic and skilled colleagues to join us as we build on existing models of care and introduce new ways of working with the changing landscape of the NHS and a greater emphasis on population health. All being delivered with the support and leadership of a forward thinking Senior Management Team. You will provide day-to-day clinical care in a supported environment and within a dynamic team. You will provide day-to-day leadership to ensure the needs of patients are placed at the centre of care delivery while assuring that standards of clinical care within a designated area are of the highest standard attainable.

You will need to demonstrate empathy and compassionate leadership alongside excellent interpersonal skills with a strong ability to educate patients and inspire colleagues through active demonstration of clinical practice. You will be willing to learn and develop within this Band 5 role.

This is an excellent opportunity for someone who has the drive to deliver the above within a well-established division that has good relationships with the emerging Integrated Neighbourhood Teams, Local authority and Local acute Trust.

Responsibilities

* To provide high-quality care to a diverse range of podiatry patients in a wide variety of settings.
* To assess, diagnose, treat, plan, and manage the podiatric (lower limb and foot care) needs of the local and transient populations to maintain patient mobility and independence.
* To work in collaboration with a team of Podiatrists and other care providers to ensure seamless care delivery; liaise with and refer as required and work under the direction of Band 6.
* To undertake appropriate clinical rotations and receive supervision and mentoring.
* To identify knowledge and skills with the manager and work towards the skills and knowledge level of a Specialist Podiatrist.

Job Description — Responsibilities

* To autonomously assess and diagnose patients with a diverse range of medical and podiatric conditions using appropriate diagnostic techniques and tools; adopt a problem-solving approach to patient care with an agreed treatment plan.
* To create and implement tailored treatment and care plans for a range of patient groups, focusing on primary foot pathologies.
* Include patients with chronic disease (e.g., Diabetes, Rheumatoid Arthritis), neurological and vascular disorders (e.g., Parkinson\'s Disease, Peripheral Vascular Disease), HIV, polypharmacy, wound management, tissue viability maintenance, terminal illness, mental health, learning difficulties, and musculoskeletal trauma.
* Engage patients and carers in their treatment plan, negotiate outcomes, gain consent, and educate on self-care.

Education/Qualification

* Essential: BSc Degree in Podiatric Medicine or Diploma (if qualified prior to 1991); Registration with the Health Professions Council; Certificate in local anaesthesia.
* Desirable: POM certificate.


Experience

* Essential: Experience of multidisciplinary and interagency working; experience with a wide range of patient and disease types including at-risk feet.
* Desirable: Experience of clinical supervision/mentoring; experience of service development.


Skills and Knowledge

* Essential: Knowledge of recent developments in the profession; podiatric theory and practice; RCOP code of conduct; equal opportunities awareness; understanding of clinical governance; up-to-date knowledge of clinical issues and treatment development.
* Desirable: Knowledge of audit.


Other

* Essential: Effective oral, listening and written communication to negotiate and gain compliance; ability to evaluate concerns and propose solutions; ability to handle sensitive diagnostic tools and surgical instruments; organizational and planning skills; experience in delivering health education.
* Desirable: IT skills (internet, e-mail, word processing).
