Job Description and Responsibilities:
1. Assessment and Person-Centered Planning:
* Conduct thorough assessments of individuals with learning disabilities.
* Develop and implement person-centered support plans, ensuring the unique needs and preferences of each service user are addressed.
2. Advocacy and Empowerment:
* Advocate for the rights of individuals with learning disabilities, ensuring their inclusion in decision-making processes.
* Empower service users to make informed choices about their lives.
3. Care Coordination:
* Collaborate with internal and external stakeholders to coordinate and monitor the delivery of specialized support services.
* Liaise with healthcare professionals, educational institutions, and community resources to provide holistic care.
4. Safeguarding:
* Identify and respond to safeguarding concerns specific to individuals with learning disabilities.
* Implement safeguarding plans in collaboration with relevant authorities.
5. Training and Support:
* Provide training and support to families, carers, and other professionals involved in the care of individuals with learning disabilities.
* Stay abreast of developments in the field of learning disabilities and share knowledge within the team.
Requirements:
- Degree in Social Work.-
- Social Work England (SWE) as a Social Worker
- Proven experience in Adult social work.
- Empathy, patience, and a strong commitment to safeguarding Children
- In-depth knowledge of UK Children Social worker legislation and policies.
