We are looking for an independent, motivated and forward-thinkingAdvanced Nurse Practitioner or Paramedic to join our growing same day care Teamwho undertake the assessment and treatment of patient needs.
We are anestablished training practice in Swindon. We have a diverse clinical teamincluding a Paramedic, Clinical Pharmacists, Admin support, Nurses & HCAswho form a strong, skilled team that are committed to providing high qualitycare and service.
Main duties of the job
You will work with our fantastic & expanding Team, supportedby GPs, a Home Visiting Paramedic, Advance Nurse Practitioner & ClinicalAdministrators to provide both in practice and home visiting on the day/urgentpatient appointments.
The main duties of the role will be patient facing clinicsincluding home visits managing minor ailments and acute conditions.
The successfulcandidate will work closely with the varied wider team of clinicians to providehigh quality care to a range of patients and support with meeting QOF and IIFtargets, participating in vaccination programmes.

Job responsibilities
The main duties of the role will be patient facing clinicsincluding home visits managing minor ailments and acute conditions, this willinclude the following:
Manage patients presenting with a range of acute and chronicmedical conditions, providing subject matter expert advice.
Request and undertake the collection of pathological specimenswhere necessary.
Process, interpret and action pathology and other test resultsas required
Keep accurate notes on patient care and ensure SNOMED CT codesare used effectively.
Recognise, assess and refer patients presenting with mentalhealth needs
Prioritise health issues and intervene appropriately.
Support the implementation and evaluation of individualspecialised treatment plans for chronic disease patients. Promoting the management and support ofpatients at risk of developing long-term conditions, preventing adverse effectsto the patients health.
Support patients in the use of their prescribed medicines orover the counter medicines (within own scope of practice), reviewing annuallyas required.
Contribute to practice targets and the formulation of documents(QOF, etc.), complying with local and regional guidance.
Support with all safeguarding matters in accordance with localand national policies. Liaise withexternal services/agencies to ensure the patient is supported appropriately(vulnerable patients, etc.)
Support the team in dealing with clinical emergencies.
Deliver opportunistic health promotion and data collection whereappropriate.
Participate in the home visiting service including ad hoc visitsto care homes.
Able to refer appropriately to secondary care or organiseemergency admissions.
Provide chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required.
