Job Introduction
We are currently recruiting for a Social Worker within our integrated discharge team to contribute to the delivery of highly effective children and adult services (within Adult Social Care) in an efficient manner. The service is delivered using a strength based, personalised approach. We work across both the acute and non‑acute sites across Medway and out‑of‑area hospitals, and we have been at the forefront of changes in the hospitals discharge‑to‑assess model. Our teamwork extends from acute settings and community settings after a period in hospital – whether in a health bed, a social care step‑down bed or in the person’s own home. The role also includes working in the emergency department of the acute hospital with health colleagues. You will collaborate closely with council colleagues and partners to ensure that person‑centred outcomes are achieved while utilising available resources as effectively as possible. You will work within organisational policy and legislative requirements, including the Care Act, Mental Capacity Act and Mental Health Act. Career Progression Framework (CPF)
Career Progression Frameworks (CPF) have been introduced for all roles at Medway Council. These frameworks provide a mechanism for employees to take ownership of their development within their post and be rewarded accordingly. Salary will be at Level A unless, based on an assessment against CPF criteria, you demonstrate that you are meeting the criteria of Level B or C, with further opportunities to progress to higher levels once in post. When writing your application, you must identify your suitability against the job profile requirements – this information will be used for shortlisting and may contribute to the CPF assessment.
