We are pleased to offer an amazing development opportunity within our All Age Mental Health Liaison Team at Tameside General Hospital.
This 12 month secondment opportunity will allow you to work within a specialist MDT covering the A&E department and acute medical wards at Tameside General Hospital. We are an all age service; seeing CAMHS, working age, later life, perinatal, substance users, supporting the best outcomes for patients, carers and their family members.
Main duties of the job
The post holder will be based in the Mental Health Liaison Team and work to support patients within the general hospital setting inclusive of the wards and A&E department who are referred for mental health assessment. The post holder will work as part of the multi-disciplinary team and undertake allocated duties within the mental health liaison team, including delivery of interventions as specified within
individual care plans. You will work under the direction of the team but be able to work without direct supervision in implementing plans of care.

Detailed job description and main responsibilities
• To provide support and promote recovery to people who are experiencing
mental health/psychological distress while an inpatient within the acute trust,
Health based place of safety and within the A&E department.
• To assist the Liaison Team practitioners in assessment, planning,
implementing and evaluating individual care plans.
• To ensure that service users engage effectively with the agreed Plan of Care.
• To work alongside colleagues within the acute trust and establish a rapport.
• To offer advice and support to non-mental health staff when dealing with
patients with mental health problems
• Provide regular and practical support for service users and their carers’ in
managing dignity and independence.
• Promote a positive attitude to the care of service users
• Provide support with activities of daily living.
• Provide information on health promotion
• Report on progress/ outcomes of care programmes to the rest of the team.
• To have the service users’ needs at the fore at all times with an underpinning
set of values and skills to promote independence and choice.
• To maintain adequate records according to Trust Record Keeping Standards,
entering appropriate details on the service users case notes as necessary.
• To attend and actively participate in training sessions, team/care planning
review meetings and supervision as appropriate.
• To contribute to the ongoing development of the service
• To ensure that electronic records are efficiently maintained.
• To contribute to audits as required, providing relevant data as required in line
with performance monitoring targets.
• To receive managerial supervision and annual IPDR
• To attend mandatory and other training identified as necessary for the post

Experience
Essential criteria
* Experience of working in mental health settings
* Experience of working with individuals in distress
* xperience of working with nonpharmacological approaches to managing challenging behaviours with people with dementia
* Experience of modelling behaviours to new staff members
* Experience of working with people with physical ill health issues and disabilities
Knowledge
Essential criteria
* Basic understanding of mental health issues including dementia.
* Understanding of confidentiality and data protection issues.
