Advanced Clinical Practitioner – Banff & Buchan cluster
An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Banff and Buchan cluster. You will deliver positive outcomes for patients by providing enhanced clinical assessment and treatment within community settings, supporting medical staff and other health care professionals, undertaking acute on‑day triage and home visits when required, and collaborating with independent practices.
Key Responsibilities
 * Provide enhanced clinical assessment and treatment for patients in community settings.
 * Support medical staff and other health care professionals in delivering optimum service.
 * Work in GP surgeries, community hospitals and patients’ own homes.
 * Undertake acute on‑day triage and home visits as needed.
 * Collaborate with independent practice teams to provide a high level of care across clusters.
 * Identify and manage deteriorating patients and initiate appropriate clinical management.
Qualifications and Requirements
 * Registered Advanced Nurse Practitioner or Advanced Clinical Practitioner.
 * Evidence of advancing practice with a sound understanding of conditions requiring hospital or virtual community ward admission.
 * Experience managing patients in their own home as part of a multidisciplinary team.
 * Completion of the Advanced Care Academy framework, including a portfolio.
 * Flexible working hours to meet service needs.
 * Valid driving licence and access to a vehicle; willingness to travel within the Banff and Buchan cluster.
