* Manage patients with common/minor/self-limiting ailments within your scope of practice.
* Offer advice to the Prescription Admin Team and address patient inquiries.
* Reconcile medicines following discharge, identifying and rectifying changes.
* Set up systems for continuity of medicines supply to high-risk patients.
* Refer patients to appropriate healthcare professionals for timely care.
* Support repeat prescribing reauthorization and identify high-risk patient cohorts.
* Contribute pharmaceutical advice for the development of new services.
* Analyze and present medicines data to support decision-making.
* Undertake clinical audits of prescribing and implement changes.
* Monitor practice prescribing against local health policies and guidance.
* Provide newsletters and bulletins on important prescribing messages.
* Deliver education and training on therapeutics and medicines optimization.
* Ensure compliance with CQC standards where medicines are involved.
* Support public health campaigns and provide specialist knowledge.
