An exciting opportunity has arrived in our Intensive Community Care (ICC) Pathway for a registered nurse or AHP currently working at band 7 or above to join our team. ICC pathway is comprised of Urgent Community Response team, Same Day Emergency Care Units, Hospital at Home, Falls Service and Single Point of Access.

This post holder will help in provision of assessment, diagnosis, treatment and management of patients and hospital admission avoidance. Our pathway services work closely together alongside other community service teams and health care providers within acute services and primary care in supporting individuals remain within their own home where safe to do so.

You will have extensive experience of working within both a community and an in‑patient setting and is a unique opportunity for an enthusiastic and committed individual to undertake a career in advanced clinical practice.

Main duties of the job

The Advanced Practitioner will be responsible for assessment, formulation and prescribing of care planning within agreed protocols. The post holder will be recognised as an expert, autonomous practitioner in an identified field of practice (including non‑medical prescribing), and will advise the multi‑disciplinary team on expert, safe and effective models of care and treatment, demonstrating a high level of practice and knowledge whilst participating in direct evidenced based patient care. They will work with a high degree of autonomy and complex decision making underpinned by a master’s level qualification that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence.

The post holder will provide clinical leadership within the team, acting as role model and source of expertise, encouraging and developing a high standard of clinical skill and improving patient outcomes. They will also participate and promote education, research, audit and service development to improve patient outcomes and quality of care delivered.

Some services operate 24 hours a day, 365 days a year, and the post holder may be required to work flexibly across shift patterns.

Detailed job description and main responsibilities

This position provides the successful clinician with an opportunity to work directly within our community services and integral to the wider Oxon Place system to test, inform and finesse the development of the urgent community response programme, playing a key role in helping older people manage long-term conditions, making sure they receive the right kind of support to help them live longer and linking in with our healthcare colleagues in the wider community.

You will be working alongside and supporting the Community Teams including the Community Therapy Service and District Nursing services however you will have the opportunity to work across many facets of Community services.

Qualifications


Essential criteria

* MSc Advanced Practice or equivalent
* Appropriate Professional qualification
* Registered with the relevant professional body
* Independent prescribing qualification
* Completing of Quality Improvement training


Desirable criteria

* Teaching Qualification (Certificate in Education/equivalent)
* Mentorship or coaching or professional advocacy qualification
* Completion of additional MSc modules


Experience


Essential criteria

* Significant clinical experience working in the NHS and other health care settings
* Extensive experience of working with patients with complex needs
* Experience of completing clinical audit and applying finding to the service
* Experience of participation in researchSignificant experience in care of patients in the specific area of clinical expertise required
* Significant experience of carrying a caseload of service users
* Significant experience of working autonomously as well as leading a multi‑disciplinary team across complex health or social care organisations ion
* Ability to support teams in challenging environments
* Ability to process sensitive and emotionally difficult information
* Ability to support clinicians and managers to problem solve issues
* Experience of delivering upsetting and/or contentious messages to patients and families


Desirable criteria

* Experience representing employing organisations at regional or national levels


Further training or job related aptitude and skills

* Ability to use a range of Microsoft IT software to present statistical data
* Demonstrable excellent communication skills, both written and verbal
* Experience of delivering upsetting and/or contentious messages to patients and families
* Ability to work across professional boundaries eg experience of working in a multi‑disciplinary team
* Good time management, organisational skills and ability to prioritise a busy/unpredictable workload
* Be able to demonstrate and understanding of the principles of equality, diversity and inclusion and how they apply to staff and patients in a health care setting
* Able to respond and prioritise unpredictable health presentations and ensure effective interventions are actioned

* Enhanced knowledge of the multi‑professional framework for Advanced Practice (2017)
* Knowledge of NHS organisations and how they work

* Understanding of evidence based practice and ability to apply to clinical work
* Working knowledge of clinical governance
* Ability to use research findings in clinical practice
* Knowledge of national standards and policy that inform best practice (for example: NICE/Department of Health guidelines)
* Knowledge of safeguarding children and vulnerable adults processes and its application to clinical practice, including support other staff
* Knowledge of leadership strategies, personal leadership and significant experience with leading successful change in an organisation
