Advanced Nurse/Clinical Practitioner - Moorlands Rural PCN
The Advanced Nurse/Clinical Practitioner (ANP/ACP) is assessed as competent in practice, utilizing expert clinical knowledge and skills. They have the autonomy to make decisions in patient assessment, diagnosis, and treatment. Working at an advanced clinical practice level, they demonstrate the necessary competencies for their specialty, including knowledge, skills, and behaviors suitable for healthcare professionals (HCP) in this role. The role aims to enable patients to receive timely care without GP referral, fostering long-term relationships and partnership with patients for optimal health outcomes.
Primary Responsibilities
The core responsibilities include managing patients with acute and chronic conditions, supporting medication use, contributing to practice targets, supporting patients at risk of long-term conditions, requesting and interpreting pathology tests, maintaining accurate records, and supporting emergency and safeguarding procedures. The ANP/ACP must practice within their professional code, exercise critical judgment, and work in partnership with patients and families.
Secondary Responsibilities
Additional tasks may include participating in local initiatives, developing specialist interests, reviewing practices, and supporting clinical service development.
