Job description
Practice Nurse
Hours: Part time x 20-28 hours per week
Reports to: Nurse Manager
Location: Issa Medical Centre or one of its branch surgeries
Length of contract: Permanent subject to satisfactory completion of training and probationary period
Summary
The role of the Practice Nurse is evolving and may grow beyond the description below to meet patient and practice needs. Responsibilities include providing nursing services using evidence-based, quality-focused care, assessing, planning, implementing, and evaluating care, and ensuring clear communication between clinical, administrative and community teams.
Teamwork is a priority; duties may be requested beyond the stated scope within your competence, including cover for sickness or absence.
Principal Responsibilities
To provide assessment, treatment, screening and health education services and advice, working in accordance with set protocols, guidelines and the NMC Code of Conduct.
Provide nursing services to patients independently or in collaboration with general practitioners according to agreed protocols.
Professional Responsibilities:
Be qualified and a member of the NMC and maintain continued education through courses and PLT events.
Able to work on own initiative, recognise limits, and refer to other health professionals when needed. Stay informed about changes in nursing care.
Use evidence-based practice to inform care, policy, and team development.
Specific Duties include (but are not limited to):
* Childhood and adult immunisations
* Cervical Smears
* Contraception/Family Planning
* Chronic disease reviews (Asthma and COPD)
* Wound management
* Blood pressure, Venepuncture
* Routine vaccinations (B12, Pneumococcal, Shingles)
* CVD risk assessments
* ECGs
* Travel vaccinations
* NHS health Checks
* Obesity and weight/diet management
* Specimen collection and testing
Additional duties include home visits for housebound patients, referrals to Podiatry, Counselling, Audiology, maintaining records, clinic management, ongoing professional development, and documentation for patient care and registration.
Attend and participate in practice and staff meetings; assist with complaints procedure and maintain appropriate professional appearance and conduct. Support health promotion campaigns and liaise with other agencies while maintaining confidentiality.
Additional training may include: Spirometry, Coil/implant assistance, INR, Prescribing, education of students.
Ensure efficient running of the treatment room and compliance with infection control policies.
Advise patients and carers on general health care and refer to GPs as necessary.
Qualification and Experience
Experience
* Essential: Understanding of computer-based medical records management; knowledge and application of supporting long-term conditions
Desirable: Worked in General Practice; experience with minor illness/injury, family planning and immunisations
Qualifications
* Essential: Member of NMC; Nursing degree or equivalent experience; willingness to learn
* Desirable: Post-registration studies in general practice; respiratory qualification
* Chronic Diseases - Respiratory: Must have experience and training in annual chronic respiratory disease reviews (Asthma and COPD)
