Responsibilities
* Promote recovery and well‑being across a variety of clinical areas, empowering service users to self‑manage and maximise independence using health coaching techniques and meaningful activity centred around individual goals.
* Work autonomously at the level appropriate to clinical competencies and within scope of practice, combining assessment, intervention and ongoing referrals.
* Provide training sessions for staff, families, carers and external agencies as required.
* Collaborate with members of the multidisciplinary team to provide proactive and reactive services.
* Contribute to the continuous evaluation of the service to ensure it is fit for purpose and increases choice through audit and research.
Role Details
The successful individual will be based in Gorleston and work throughout the Great Yarmouth & Waveney area. They will join an enthusiastic and supportive multidisciplinary neurology team in providing high‑quality care and rehabilitation. The role requires experience in supporting people with progressive neurological conditions and offers a strong commitment to clinical supervision, continuing professional development and opportunities for supervision, training and peer support. Collaboration will be close with Stroke Early Supported Discharge and primary care home colleagues.
