Description & Requirements

We have an exciting full time opportunity for you to become our Specialist Health & Wellbeing Coach (Nutritionist).

The role is hybrid and the office is based in Hatfield so you need to drive and have your own transport. The role does involve supporting families so experience of working with children/young people is ideal. The role will also involve working at least 3 evenings a week with the latest delivery being until 19:30.

The Specialist Health & Wellbeing Coach provides individually tailored diet, nutrition, and behaviour change advice to service users / participants (adults and/or children, young people, and families) to manage their weight and make sustainable changes to improve their overall health and wellbeing.

As a Specialist Health & Wellbeing Coach you will provide evidence-based dietary and behaviour change services via one-to-one and group-based service deliver and ensure operational safety of weight management group programmes. As part of this you will support your caseload to implement experiments, track behaviours, utilise tech enabled support, make best use of resources, support each other, explore / use wider community provision and achieve their outcomes.

To provide medium and high need nutritional and behaviour change services for eligible participants, empowering children, families, and adults to manage their weight and lifestyle-behaviours effectively and support service users / participants in achieving their own self-identified health and wellbeing goals.

Depending on need participants will experience either virtual or face-to-face health coaching group delivery sessions in the community, including but not limited to:

1. Supporting the recruitment of individuals to the interventions
2. Support our triage and assessment process.
3. Agenda setting with clients.
4. Lifestyle behaviour change support to move more, achieve a healthy weight/diet.
5. Problem solving with adaptability to individual context.
6. Motivational interviewing
7. Goal setting
8. Signposting to other services if required (e.g., stop smoking, reduce alcohol consumption levels)

Delivery group weight management that is underpinned by behaviour change methodology and empowers service users / participants to manage their weight and lifestyle-behaviours effectively achieving their own self-identified health and wellbeing goals, ensuring information, advice and guidance is:

To deliver services in line with quality standards, national guidance and within own competencies. If a situation is beyond the skillset of the post-holder, then they should identify when further work with a client is inappropriate and escalate appropriately or signpost to other services (e.g. tier 3 weight management).
Support with outreach community engagement work inclusive, but not limited to:

20. Screening services (e.g., NHS Health Checks)

Work in line with business and contract performance requirements including:

24. Implementing Standard Operating Procedures (SOP)
25. Fulfilling policies and quality management framework expectations including (but not limited to), data protection, information security, Health and safety and safeguarding.
26. Participate in regular audits and performance observations, with a pro-active commitment to providing high quality services and continuous professional development.
27. A proactive approach to supervision and training with a demonstrated commitment to personal development, achievement of quality standards, high performance in line with agreed objectives and Key Performance Indicators KPIs.

To ensure accurate reporting and data collection in line with contractual and case management system requirements to ensure:

28. Caseloads can be managed efficiently, effectively and at scale.
29. Accurate Data to be entered in a timely manner.
30. Team members are able to support in population behaviour change as required and in line with need.
31. Wider informal and formal care givers / specialists can complement and enhance the behaviour change journey.
32. Service users / participants only have to tell their story once and where behaviour changes occur sequentially or concurrently, we are able to offer a comprehensive and coordinated supportive services.
33. Data audits accurately capture activity, productivity, utilisation, and outcomes.


Note: This is not an exhaustive list. The Specialist Health & Wellbeing Coach is expected to carry out all other duties as may be reasonably required.

Salary: Non-London £25700 - £28800

London £28300 - £31300

Qualifications & Experience


Essential

34. AFN registered degree in Nutrition or HCPC registered degree in dietetics with portfolio evidence of continuous professional development
35. Experience in delivering group-based behaviour change interventions to child, families or adults. Evidence of supporting vulnerable individuals through a change process
36. Experience of caseload management demonstrated via the use of a Case Management System
37. Experience of lesson planning and nutritional / weight management content creation
38. Experience of inputting and processing data where accuracy, targets and time deadlines are operating in the workplace
39. Evidence of continuous professional development to ensure the delivery of the most recent and up to date health coaching provision to the correct quality standard
40. Experience of working in a health, community or social care environment or good understanding of the issues around maintaining a healthy lifestyle Desirable:
41. A health coaching qualification or an accredited health coaching skills programme.
42. Experience of direct health coaching delivery
43. Experience in delivering behaviour change interventions for specific lifestyle related issues (e.g., smoking cessation, physical activity, NHS Health Checks, weight management, alcohol reduction)
44. Evidence of working with individuals with long term health conditions
45. Experience of working in community settings
46. Experience of managing own caseload
47. Experience of supporting people with additional needs

Individual Competencies


Essential

48. A strong understanding of the social determinants of health
49. A strong understanding of population-based approaches including segmentation and risk stratification. A strong understanding of behaviour change principles and methodology including the COM-B model and behaviour change wheel
50. Excellent oral and written communication skills with people from a wide variety of backgrounds
51. Good presentation skills and ability to work effectively with individuals and groups (adults and/or children).
52. Good understanding of principles of confidentiality and safeguarding
53. Demonstrate core skills and competencies as set out in best practice standards including:
54. Practice in line with professional conduct and practice
55. Describe the relationship between different metabolic pathways and how these might differ depending on individual context
56. Describe what nutrients are and how they are used by the body
57. Explain the nature of common conditions that require dietary manipulation or affect physical activity e.g. obesity
58. Understand, appraise and communicate latest nutritional research
59. Excellent internal and external stakeholder engagement and management
60. Good understanding of principles of confidentiality and safeguarding
61. A strong track record of interpersonal relationships including the ability to build, develop and maintain relationships
62. Effective caseload management inclusive of accurate data collection, data entry, timely recording, and delivery against agreed Service Level Agreements (SLAs) and Standard Operating Procedures (SOPs).
63. Confident in the use of evolving digital technologies to support people through behaviour change processes. The ability to manage time independently and effectively and work to deadlines
64. Effective safe and sensitive data management in line with information security standards
65. Excellent Microsoft Office skills and ability to learn quickly when using news systems and processes
66. Commitment to personal development and training
67. Commitment to flexible working (including evenings and weekends), ensuring the service is available to all residents including, those who work during the day, shift workers and those with children of school age
68. Commitment to supporting overall team/contract performance to the highest standards, undertaking a range of duties in line with business requirements
