Community Nurse – Learning Disabilities – Band 6. This advert is for posts available across the city at our 3 bases: Kings Norton, Small Heath and Erdington. Preference of base will be discussed at the offer stage.
An exciting opportunity has arisen for an experienced and highly motivated individual to join the community city-wide service within the Learning Disabilities Service. You will be an experienced and flexible individual with excellent communication skills and a passion for multi-disciplinary team working. To work within the framework of a multi-disciplinary team to provide support and guidance to children and adults with learning disabilities and their carers. To work within the framework of health facilitation (level one and two). To co-ordinate a clinical service under the supervision of Senior Community Nurses. Be Part of Our Team.

Responsibilities
* Manage and co-ordinate a caseload under the supervision of the Senior Community Nurse.
* Focus on individual health outcomes for people with learning disabilities and offer support to access health services.
* To work within the framework of a multi-disciplinary team to provide support and guidance to children and adults with learning disabilities and their carers.

Posts available across the city at Kings Norton, Small Heath and Erdington. Preference of base will be discussed at the offer stage.
Requirements and Qualifications
* Experienced and highly motivated individual with excellent communication skills.
* Ability to work effectively within a multi-disciplinary team.
* Experience in community settings; health facilitation (level one and two).
* Co-ordination of clinical services under supervision of Senior Community Nurses.
