The Role
An opportunity has arisen for 2 Social Worker posts within the Intermediate Tier Services, one of which will be a Social Care Navigator role. This service covers Admission Avoidance; Intermediate Care and Discharge to Assess bed-based units.
This post offers an opportunity to contribute to the ongoing development of innovative ways of working across health and social care in order to improve the experience of service users and carers and to promote well‑being and independence.
Develop, maintain and improve relationships with service users and carers is essential in this role as well as the attributes required to participate in inter-disciplinary team working.
The role requires some weekend working on a rota basis to cover the Admission Avoidance Team duty function and also bank holidays on a rota basis. The Social Care Navigator receives a 7% enhancement to support 365‑day service delivery.
Responsibilities
* Contribute to the development of innovative ways of working across health and social care.
* Develop, maintain and improve relationships with service users and carers.
* Participate in inter‑disciplinary team working.
* Carry out assessments and reviews of need under the Care Act 2014.
* Assist with support planning and link people to universal and voluntary services.
* Provide support with complex cases, mental capacity and best interest assessments.
* Provide support to colleagues in other areas of the services as required.
Qualifications & Requirements
* Social Work qualified Diploma/Degree and SWE registered.
* Experience working with adults.
* Strong communication and interpersonal skills.
* Commitment to safeguarding and promoting the welfare of vulnerable adults and young people.
Additional Information
Disability and criminal record checks will be required for this role. The post is a customer‑facing role under the fluency duty and requires a specified level of spoken English.
