Advanced Clinical/Nurse Practitioner (ACP/ANP)

In this role you will provide high-quality, patient-centred care to people in their own homes across York. This is a rewarding opportunity to work autonomously, support local GP practices, and deliver excellent clinical care to patients who need it most.
Role Overview:
* Conduct home visits for acute and chronic conditions
* Perform assessments, diagnoses, and management plans
* Prescribe safely and appropriately
* Work collaboratively with local GP practices and community services
* Deliver compassionate care to patients who may be frail, housebound, or unable to attend appointments
Required Qualifications & Experience
* NMC/HCPC Registration
* Master’s (MSc) in Advanced Clinical Practice or equivalent Level 7 qualification.
* V300 Independent Non-Medical Prescribing
* Experience in primary or urgent care
* Strong clinical decision-making skills
