Main duties of the job

We are looking for a SPDN Community Clinical Nurse Specialist to join our Community Nursing Team in Faversham.

The Community Nursing service will complete holistic assessment, plan appropriate care and delivering clinical interventions to meet the various needs of housebound patients within your geographical area. Nurses will offer health education advice and promote independence in all consultations to support patients to manage their complex long-term conditions appropriately and maintain their independence.

The role is responsible for ensuring appropriate allocation of staff resources on the duty roster, managing annual leave and ensuring staff remain compliant with mandatory and essential training. There is a need to ensure that capacity and demand for the service is managed effectively and concerns are escalated in the appropriate manner.

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Person specification

Qualifications, Training and Experience

Essential criteria

1. You’ll need to have a Level 6 Degree in Nursing or equivalent knowledge acquired through experience plus a diploma in nursing
2. SPDN course
3. You’ll need to have experience within your area of practice
4. You will be a registered health care professional with the Nursing and Midwifery Council
5. Full UK/EU driving licence

Desirable criteria

Essential criteria

7. You will need to be able to be able to use IT systems and databases
8. You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks
9. You will fully understand the role and remit of a health care worker.
10. You will know when you are within your own limits and when to escalate

Desirable criteria

11. Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks and NICE guidance
