Job overview

Community Nursing Teams provide interventions for housebound
patients aged 18 and over delivering personalised nursing care to
those with complex and long-term conditions. The aim of our service is to enable patients to stay safe and well in their chosen environment.

The service actively works to prevent avoidable admissions by providing tailored care that meets the patient need and assists them in actively achieving their long-term health goals and self-care where possible. The service operates in teams aligned to Primary Care Networks and provides integrated care founded in the framework of the Multidisciplinary Team

Main duties of the job

Your daily activities will be varied to include:

1. Palliative and end of life care provision
including symptom control, advance care
planning and verification of death
2. Administration of medication including IV
therapy
3. Wound management including leg ulcers,
post-operative surgical wounds, drains
4. Bladder and bowel care
5. Health promotion, education and support to
manage long-term conditions

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB).

Job responsibilities

As a community Trust many of our roles require individuals to be able to drive to perform their duties. As such you will be asked to confirm you have a full UK driving licence where driving is a requirement of the role to be able to proceed with your application. Person specification

Qualifications, training and experience

Essential criteria

6. You’ll need to have a Level 6 Degree or equivalent qualification in relevant subject Nursing
7. You will be a registered health care professional with the Nursing and Midwifery Council.
8. You’ll need to have recent experience of healthcare gained through education, work experience or previous employment. This role is ideal for a newly qualified graduate nurse.
9. Full UK Driving License with a car

Knowledge and skills

Essential criteria

10. You’ll need have sound communication skills in order to understand and communicate both verbally and in writing.
11. Having an awareness of developments within the NHS would be an advantage.
12. You will need to be able to work well under pressure and be able to organise your own work.
13. You will need to be able to be able to use IT systems and databases.
14. Work together with colleagues and other teams across the Trust.
15. represent the Trust when talking to and working with patients, colleagues and visitors.

Desirable criteria

16. You will understand where to signpost clients and families to alternative services to support behaviour change and improve health.
17. You will fully understand the role and remit of a health care worker.
18. You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks.
19. You will need to have hand and eye coordination and dexterity to use your clinical skills.
20. You’ll need to be adaptable in work as things change as part of working in a wider team.

Working environment

Essential criteria

21. The role is physically demanding and can include standing for long periods of time. You will move and handle patients, equipment and other physical health care tasks.
22. We encourage a culture where colleagues can be open and transparent about their own practice, team working to create a culture of continuous improvement and sharing good practice in order to improve care.
23. A full, valid UK driving licence and the ability to travel independently are essential for this role. Should you require workplace adjustments under the Equality Act 2010, we are committed to supporting you to meet the mobility requirements of the role wherever possible.
24. Tact and sensitivity will be essential in this role. There will be times when you see distressing situations which may include vulnerable, violent or aggressive patients. You will need to adapt your style to provide reassurance and empathy.
25. Due to the nature of the role, you will have frequent exposure to fluids from the body and clinical waste. You will also be required to concentrate on providing clinical care where there may be unexpected interruptions.
