We are looking for a friendly and approachable candidatewho ispassionate about supporting individuals to improve their health and wellbeing.An ability to work well both within a team and autonomously is essential aswell as experience in supporting both mental and physical health needs.

The successful candidate will support patients across the PCN to providehealth and wellbeing support interventions. HOSMS PCN will provide supportwhich will include supervision and mentorship. Experience in general practiceor healthcare settings would be an advantage but is not essential. Moreimportant for this exciting role is a positive attitude and expertise inengaging and motivating people, using health coaching techniques to supportthem towards behavior change.


Main duties of the job

Triage and assess patients

Manage and prioritise a patient list

Provide 1:1 health coaching

Use a number of tools to empower patients to takecontrol of their own health

Provide interventions such as coping skills,self-management and education relevant to the individual

Coach and motivate patients

Facilitate group health coaching interventions such asHOPE Programme

Encourage patients to actively participate in theirrecovery journey

Provide personalised support to individuals, theirfamilies, and carers to support them to be active participants in their ownhealthcare; empowering them to manage their own health and wellbeing and tolive independently

Increase patient motivation to self-manage and adopthealthy behaviours

Refer people back to other health professionals withinthe PCN when appropriate

Support personal choice and positive risk taking

Work in partnership with the social prescribingservice to connect patients to community-based

Network with VCSE organisations

Undertake project work alongside triage andmaintenance of a 1-2-1 caseload.

Communicate information in a way that makes itrelevant and understandable

Actively contribute to and developmulti-disciplinary team working processes

Facilitate fair access to social care services andpromote social inclusion for service users aged 18yrs and over, ethnicity,gender, sexual orientation or disability


About us

HOSMS Primary Care Network is located in East Devon and has a population of around 50,000 patients. There are 3 surgeries within the PCN, with excellent inter-personal relations, a good support network and a history of collaborative working. The practices are Honiton Surgery, Coleridge Surgery and Sid Valley Practice.

The PCN has a number of teams which work across the three practices, including the Pharmacy Team, Health and Wellbeing Team and FCP Physiotherapy Team. The teams are supported by a Lead Manager, Deputy PCN Manager, Clinical Director as well as clinical supervision from other members of the three practices.

We are a forward thinking, friendly network and we pride ourselves in offering our patients the best care possible, demonstrating innovation, clinical excellence and working collaboratively with our health care professionals across the network.


Job responsibilities

HOSMS Primary Care Network have an exciting opportunity for a Health andWellbeing Coach to join their successful PCN Health and Wellbeing Team. Thisestablished team has been running health and wellbeing services includingHealth and Wellbeing Coaching, Social Prescribing, Group Work, Signposting andStaff Wellbeing Support for over five years across Honiton Surgery, ColeridgeMedical Centre & Sid Valley Practice.

JOBRESPONSIBILITIES:

Triage and assess patients under the guidance of thePCN Clinical Lead

Manage and prioritise a patient list

Provide 1:1 health coaching interventions both inpractice and virtually for patients identified as having mild to moderatemental health needs, physical health needs or requiring support with behaviourchange to improve their health.

Use a non-judgemental, strengths and recovery basedapproach which considers the person as a whole when addressing their healthissues

Use a number of tools to empower patients to takecontrol of their own health

Provide interventions such as coping skills,self-management and education relevant to the individual

Coach and motivate patients through multiple sessionsto identify their needs, set goals and support them to achieve theirpersonalised health and care plan objectives.

Facilitate group health coaching interventions such asHOPE Programme

Encourage patients to actively participate in theirrecovery journey using a working knowledge of community assets, increasingtheir ability to access and utilise community support offers

Provide personalised support to individuals, theirfamilies, and carers to support them to be active participants in their ownhealthcare; empowering them to manage their own health and wellbeing and tolive independently

Increase patient motivation to self-manage and adopthealthy behaviours

Refer people back to other health professionals withinthe PCN when appropriate

Support personal choice and positive risk takingwhilst ensuring that patients understand the accountability of their ownactions and decisions, thus encouraging the proactive prevention of furtherillnesses

Work in partnership with the social prescribingservice to connect patients to community-based

RESPONSIBILITY FOR ADMINISTRATION:

Other duties and responsibilities to be undertaken may include any or allthe items in the following list

Taking messages and passing on as appropriate

Computer data entry/data allocation andcollation, processing and recording information in accordance with practiceprocedures

Initiating contact with and responding torequests from patients, other team members and associated healthcareprofessionals and providers

Helping to keep all practice areas tidy and freefrom obstructions and clutter

Helping to maintain a positive, supportiveculture across the whole of the practice team

Any other duties commensurate with this position

Duties will vary from time to time under the direction of thePartners/Strategic Manager dependent on current and evolving practice workloadand staffing levels.

ANALYTICAL AND JUDGEMENT SKILLS:

Participatein risk assessments and determine the appropriate course of action, taking intoconsideration a range of possible options. Where necessary, escalating risk toclinical lead safeguarding leads or other relevant services.

Havean awareness of your own competencies, knowledge base and experience, stayingwithin scope of practice and seek support as required.

PLANNING AND ORGANISATIONAL SKILLS:

Responsible for managingown workload to ensure the needs to the individuals who use our services aremet, taking the wider needs of the team into consideration

Ensure that interventionsare planned in an integrated and co-ordinated manner, this may include liaisonwith external agencies

Plan and organiseindividuals packages of care and ensure that reviews are held within therequired time frames

Plan their own workloadand support the workload of other members of the multi-disciplinary team toensure that care is provided in the most high quality manner.

CONFIDENTIALITY:In the course of seeking treatment,patients entrust us with, or allow us to gather, sensitive information inrelation to their health and other matters. They do so in confidence andhave the right to expect that staff will respect their privacy and actappropriately. In the performance of the duties outlinedin this job description, the post-holder may have access to confidentialinformation relating to patients and their carers, surgery staff and otherhealthcare workers. They may alsohave access to information relating to the surgery as a businessorganisation. All such information from any source is to be regardedas strictly confidential

Information relating to patients, carers, colleagues, otherhealthcare workers or the business of the surgery may only be divulged toauthorised persons in accordance with the surgery policies and proceduresrelating to confidentiality and the protection of personal and sensitivedata

HEALTH & SAFETY:

The post-holder will assist inpromoting and maintaining their own and others health, safety and security asdefined in the surgery Health & Safety Policy, the surgery Health &Safety Manual, and the surgery Infection Control policy and publishedprocedures. This will include:

Using personal security systems within theworkplace according to Surgery guidelines

Identifying the risks involved in workactivities and undertaking such activities in a way that manages thoserisks

Making effective use of training to updateknowledge and skills

Using appropriate infection controlprocedures, maintaining work areas in a tidy and safe way and free fromhazards

Actively reporting of health and safetyhazards and infection hazards immediately when recognised

Keeping own work areas and general /patient areas generally clean, assisting in the maintenance of generalstandards of cleanliness consistent with the scope of the job holdersrole

Undertaking periodic infection controltraining (minimum annually)

Person Specification


Experience

* Experience of working as part of a team
* Working with vulnerable adults
* Experience supporting individuals to improve their health and wellbeing.
* Experience of delivering lifestyle changes interventions
* Working in a multi-disciplinary setting
* Working within healthcare settings.
* Working with or within community settings to support health & wellbeing.

Qualifications

* Educated to GCSE level or equivalent
* Health & Well-being qualification or working towards or
* Undertaking or qualified from appropriate health coaching training covering topics outlined in the NHS England and NHS Improvement Implementation and Quality Summary Guide
