About the Role:
* Provide specialist social work support to adults with learning disabilities and autism, promoting their independence, well-being, and inclusion within the community.
* Conduct comprehensive needs assessments and develop person-centered care plans tailored to the unique needs of each individual.
* Collaborate effectively with individuals, their families, carers, and other professionals to achieve positive outcomes.
* Advocate for individuals' rights and access to appropriate services and support.
* Contribute to a supportive and collaborative team environment focused on best practice and positive change.
Requirements:
* Registered with Social Care England.
* Minimum 3 years' post-qualification experience as a Social Worker.
* Significant experience working with adults with learning disabilities and/or autism is essential.
* Excellent communication, interpersonal, and case management skills. Experience with challenging behaviour is highly desirable.
* Commitment to person-centered care and ethical practice.
