Overview
Practice Nurse - Primary Care role at NHS. Location: Seven Dials Medical Centre, Brighton.
Opportunity for a registered Practice Nurse with Primary Care experience to join a clinical team and manage day-to-day running of the treatment room, organise clinics, and provide high quality nursing care. Focus includes preventative care, health promotion and management of long-term conditions across all ages.
Responsibilities
* Implement and evaluate individual treatment plans for chronic disease patients
* Identify, manage and support patients at risk of developing long-term conditions, preventing adverse health effects
* Provide routine nursing care in line with clinical evidence, NICE guidelines and the NSF
* Provide wound care (e.g., ulcers, Doppler) and chronic disease clinics
* Refer patients to secondary or specialist care as required
* Maintain accurate clinical records in compliance with legislation
* Maintain chronic disease registers
* Recognise, assess and refer patients with mental health needs
* Implement vaccination programmes for adults and children
* Support safeguarding within the clinical team in line with local and national policies
* Coordinate nursing services for the practice and delegate duties within the nursing team as appropriate
* Provide general health screenings within agreed protocols and refer to the care team as necessary
Qualifications and Skills
* Essential: Registered Nurse (NMC) with evidence of practicing at the relevant level as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework
* Broad knowledge of general practice and experience as a Practice or Community Nurse
* Experience with chronic disease management and autonomous working
* Experience in infection prevention and control; caring for elderly and housebound patients
* Experience with nursing skills including wound care, ECG, venepuncture, immunisations, new patient medicals, women’s health (e.g., cervical cytology, contraception)
* Ability to interpret pathology tests, provide patient guidance, and promote best practice
* Strong IT skills and ability to record accurate clinical notes
* Effective communication skills with patients by telephone, email and in person
* Understanding safeguarding adults and children; knowledge of GP surgery provision, QOF and enhanced services
* Ability to work within own scope and know when to refer to GPs; good time management
* Ability to follow legal, ethical and professional policies and codes of conduct
* DSB check, Occupational Health clearance, current UK registration
* Commitment to ongoing professional development and CPD
* Flexibility to work outside core office hours
* Desirable: Family planning qualification; experience with quality initiatives; knowledge of NHS landscape, clinical governance, public health locally
* Awareness of health promotion strategies and national standards informing general practice (NSF/NICE)
* Personal accountability, emotional resilience, and ability to work under pressure
* Access to own transport and ability to travel within the locality

Seven Dials Medical Centre serves just over 7,000 patients in Brighton and Hove with 14 staff including 3 GP Partners, an ACP, a Practice Nurse and HCA. The team works closely with the PCN to offer access to Social Prescribers, Mental Health Co-ordinators, Care Co-ordinators and a Pharmacy team. The practice aims to deliver high-quality care and maintains a family GP feel, with many patients registered for decades. The environment is friendly, welcoming and professional.
