Were recruiting for two part-time roles one in Herts Five PCN and one in Potters Bar PCN. Our communities are wonderfully diverse, ranging from rural and suburban areas to some of the most deprived neighbourhoods in Hertsmere. That means no two days are ever the same and your work will truly change lives.


Main duties of the job

What youll do:

Meet with patients in GP surgeries, in their homes, or by phone to understand what matters to them.

Create personalised support plans that connect people to local groups, activities, and services.

Build strong relationships with community organisations and help them thrive.

Support people facing challenges such as isolation, long-term health conditions, or financial/housing difficulties.

Work alongside GPs, nurses, and other health professionals as part of a multidisciplinary team.

You might already work in health, social care, or the voluntary sector or you might come from a completely different background. What matters most is that you are:

* Empathetic, approachable, and a great listener.

* Organised and confident working independently.

* Able to build trust quickly and work with people from all walks of life.

* Willing to complete the required training and keep learning.

Job responsibilities

Purpose of the Role

Social prescribing helps people improve their health and wellbeing by connecting them with non-medical support in their community. Link Workers give people time, focus on what matters to them, and take a holistic approach to care tackling issues such as isolation, housing, debt, or lifestyle changes alongside any health concerns.

As a Social Prescribing Link Worker, you will:

* Work as part of the GP practice team and wider PCN network.

* Support patients to take control of their wellbeing, live independently, and improve health outcomes.

* Build strong partnerships with community groups, voluntary organisations, and statutory services.

Key Responsibilities

1. Patient Support

* Receive referrals from GPs, practice teams, and other agencies.

* Meet patients in GP surgeries, community settings, or their own homes.

* Listen, understand individual needs, and focus on what matters to them.

* Co-produce personalised care plans to improve health and wellbeing.

* Support people to access community groups, activities, and services sometimes attending with them for the first time.

* Maintain contact to ensure people feel supported and able to sustain changes.

2. Community Development
* Build and maintain a directory (map) of local groups and services.

* Support local voluntary and community sector (VCSE) organisations to receive referrals safely and effectively.

* Help strengthen community groups by signposting to funding or resources.

* Identify gaps in local provision and work with partners to develop new services.

3. Partnership Working
* Work closely with GPs, nurses, practice staff, and other health professionals.

* Attend PCN meetings and contribute to local health and wellbeing projects.

* Promote social prescribing to local agencies and encourage appropriate referrals.

* Build trust with community partners and statutory services to improve joined-up support.

4. Administration & Governance
* Manage your caseload using primary care systems (EMIS).

* Keep accurate, confidential records of patient contacts and progress.

* Monitor and report on outcomes, including patient feedback and case studies.

* Work within safeguarding, data protection, lone working, and health & safety policies.

* Access regular clinical supervision and take part in training and peer support networks.


Person Specification


Experience


Essential

* Working in community development, public health, social care, primary care or similar (paid or unpaid)
* Supporting individuals or families in a related role


Desirable

* Experience supporting people with mental health needs. Experience working with VCSE sector. Partnership working with multiple agencies.

Essential

* Empathetic, approachable, and non-judgemental. Able to build trust and work with diverse communities. Organised, proactive, and resilient. Commitment to reducing health inequalities.


Desirable

* Lived experience relevant to the communities served.


Skills and Knowledge


Essential

* Understanding of the wider determinants of health. Strong communication skills (verbal and written). Ability to work independently and manage a caseload. Basic IT skills (email, word processing, online systems).


Desirable

* Knowledge of personalised care approach, community development, or local services. Motivational interviewing skills.


Qualifications


Essential

* NVQ Level 3 Social Prescribing Link Worker or equivalent or commitment to work towards.
* Commitment to continuing professional development


Desirable

* Training in motivational interviewing, coaching, or similar
