Job Summary
Working with the Community Falls Service team, you will provide person‑centered occupational therapy to help people stay safe, independent and confident after a fall. You will support individuals to make informed choices about daily activities, improve wellbeing, and work closely with colleagues across therapy, nursing and wider services.
Key Responsibilities
What you'll do:
* Assess people's needs and plan safe, effective occupational therapy interventions
* Deliver therapy programmes in homes and community settings
* Provide clear, supportive advice to individuals, families and carers
* Monitor and review progress, adjusting plans as needed
* Contribute to service development, research and quality improvement
Qualifications
Essential:
* Diploma or Degree in Occupational Therapy
* Current HCPC registration
* Skills in assessment, communication and planning care
* Basic IT skills and understanding of relevant legislation and clinical guidance
* Ability to manage own workload and adapt to changing priorities
* Willingness to learn, take part in supervision and develop practice
Desirable:
* Experience as a Practice Educator of preregistration students
* Evidence of current CPD or advanced qualifications at postgraduate level
* Understanding of policy, legislation drivers and their application to clinical practice
* Knowledge of occupational therapy models of practice and standardized assessments
* Experience working in community and adult physical health clinical practice areas
* Strong inter‑personal/communication skills, time management, and caseload management experience
* Evidence of specialist practice skills and impact on practice change/development
* Evidence of promoting user involvement and participation
