We are seeking to appoint a motivated Rehab Support Worker to join the Primary Care Home Hub (PCH) based at Martham Healthcentre at Great Yarmouth. The team delivers Community healthcare to patients in their own home in the Great Yarmouth and Northern Villages area.

This role focuses on the provision of reablement interventions. Delivering care, support and encouragement to patients in order to promote independence. Responding to patient and family needs. Prevent unnecessary hospital admission and support end of life care at home. The Primary Care Home is a multidisciplinary team of Nurses, Therapists, Assistant Practitioners, Health Care Assistants, and Rehabilitation Support Workers delivering nursing, therapy and reablement/rehab services to adults in their own homes.

Do you have excellent communication skills, consider yourself to be caring and compassionate and want to make a difference to the patients experience on a daily basis, whilst being able to demonstrate how you embody our values and commitments and have a desire for personal and professional develop.

Ideally some experience in community work would be an advantage however consideration would be given to the right candidate.


Main duties of the job

* To work with patients towards self-care and independence.
* To support and deliver aspects of activites of daily living. To include management of dietary and fluid intake, management of continence, assisting with toileting, including use of sheaths, catheters and continence pads.
* Take, record and report blood pressure, respiration, pulse rate, oxygen saturation levels, urinalysis. Be able to recognise and report the deteriorating condition of a patient
* Undertake skin assessments. Undertake like for like dressing change if required.
* Provide walking aids, provide and fit small aids and equipment as directed.
* Prompt, assist and manage medication as directed by Medication Administration Record chart. Reporting any problems with medication and/ or compliance
* Participate in caring for patients at the end of life when appropriate.
* Ensure a high standard of record keeping is achieved in line with ECCH standards.
* The service uses SystmOne as a clinical IT system to support mobile working.

The service operates 7 days a week 7:00 to 20:30 hours. Applicants will be required to work a variety of shifts including weekends and bank holidays. At times you may be required to work with another of the 4 PCH's from a different location to cover staff sickness, or capacity pressures. If not previously achieved you will undertake the nationally recognised Care Certificate programme on commencing employment.

This role attracts paid enhancementsfor out of hours, bank holiday and weekend working.

Job responsibilities

* Contribute to, supportand promote ECCHs, strategic direction, values and culture in relation to primarycare home services.
* Communicateeffectively with patients, relatives and carers, respecting patient confidentialitywith privacy and respect for diverse cultural backgrounds and requirements.
* Communicate routineinformation to patients, relatives or carers, using skills of persuasion,reassurance, tact and empathy as required.
* To support relatives atall times and be aware of the effects of illness on the family.
* Enable individuals andtheir families to express their needs and preferences and take appropriateaction to overcome barriers to communication.
* To support and deliveraspects of activites of daily living. To include management of dietary andfluid intake, management of continence, assisting with toileting programmes,including use of sheaths, catheters and continence pads.
* To promote confidencein the patient by means of appropriate standards of appearance, behaviour andattitudes
* Check compliance withmedication- Prompt, assist and manage medication as directed by MedicationAdministration Record chart. Reporting any problems with medication and/ orcompliance.
* To safeguard theconfidentiality of all personally sensitive information.
* Contribute to theclinical governance agenda through participation in clinical audit.
* Create an environmentconducive to effective working, respecting and supporting staff to deliver highquality clinical services.
* Work as part of themultidisciplinary Primary Care Home to deliver planned and unplanned services.
* Work with patientstowards self-care and independence.
* Participate in caringfor patients at the end of life when appropriate.
* To implement the careas planned taking into account own competencies.

* Take responsibility forthe safe use of clinical equipment,

* Report any concerns/changes in a patients condition to a registered health care professionalwithout delay.

Qualifications
