We have an exciting opportunity for a highly motivated Senior Health Care Support Worker to join theCommunity Learning Disability Health Team in Barnsley.This is a full-time Band 3 permanent post.
The Team consistsof a comprehensive multi-disciplinary team including Learning DisabilityNurses, psychiatrists, clinical psychologists, occupational therapists,physiotherapist, speech & language therapists, dietician, therapyassistants and healthcare support workers. The Team works together to supportindividuals aged 18 and over with diagnosed learning disabilities and theirfamilies/carers
Atthe time of advertising, this role is eligible for sponsorship under the Healthand Care Worker visa. However, sponsorship can only be offered if the proratedsalary meets the minimum salary threshold of £25,000 set by UK Visa andImmigration.

We are looking for a compassionate, excellentcommunicator as a Senior Support worker, who can work under the supervision ofthe nursing team, to deliver plannedprogrammes of care to people with learning disabilities. Examples include,supporting service users to understand their health needs, promoting annualhealth checks, completing hospital passports. You would support service usersto health appointments when appropriate, support their understanding inappointments, and provide health education, make and use resources, e.g.: easyread. You will work with carers and families, in different care settings, youwill have a case load under the supervision of a named nurse, you need to beflexible, organised and able to prioritise your workload in the best interestof the service user.
You will be expected to engagein management and clinical supervision.
Car access is essential.

Assess situations, for eg, following the observation policy,identify and resolve potential problems within own levels of competence.
feedback theirobservations to the registered professional / clinical team to contribute tothe care planning process. Understand the principles underlying care planningeg, enabling the service user to reach an optimum level of functioning/independence/recovery.
implement care withinwell-established policies, procedures, protocols, guidelines and establishedclinical care plans. apply basic knowledge of longer-termhealth conditions, within the remit of care planning, providing advice, supportand feedback to a registered professional.
Undertake the physicalaspects of care eg, moving and handling of service users. Use items ofequipment in delivering care eg, hoists, wheelchairs etc. (Supportservice users with personal care duties (bathing, toileting, dressing, supportwith meals, assist patients with their appearance), as part of ahealthcare team, provide the highest standards of safe and compassionateplanned care to people that maximises their recovery and rehabilitationpotential.
Undertakesa limited range of delegated clinical care duties relevant to the serviceprovided/work area and records patient observations: e.g. taking bloodpressure, blood glucose monitoring, pregnancy testing, observing patients forsigns of agitation or distress, removal of peripheral cannula urinalysis,removal of catheter.
participate in a flexiblerostering arrangement providing 24-hour/7 day care (as appropriate to the role)

GCSE A-D or grade 3-9 in English and Maths or equivalent.
Level 2 qualification/or working through, with the ability to demonstrate competencies at level 3.
Care Certificate or required to achieve within 12 weeks of commencement.
Apprenticeship level 3 or equivalent eg, RQF(regulated Qualification Framework)NVQ level 3 in Health & Social Care or equivalent.
In providing personal care and related procedures for those with health/social needs.
Evidence of a commitment to user/carer involvement.
A satisfactory sickness record over previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where sickness is related to a disability and/or pregnancy).
Able to fulfil Occupational Health requirements for the post (with reasonable adjustments if necessary).
A Current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).
